The indictment includes three counts of Unlawfully Obtaining Benefits or Payments from a Medical Assistance Program, along with one count each of Aggravated Grand Theft, Perjury to Obtain State Benefits, and Failure to Keep Necessary Records Upon Which a Medicaid Claim is Based.
